Your First Visit
Upon initial arrival, you’ll be greeted by our kind front desk team member. From there, you’ll receive all the new patient forms you need to complete. After just a few minutes, a team member will call you in so they can bring you to one of our dedicated operatories. You’ll get a chance to meet with Dr. Aptaker who will then discuss your new patient forms as well as answer any questions. After a brief exam, you’ll be scheduled for a biannual checkup before you leave.
New Patient Forms
When you fill out new patient forms, our team is better able to meet your dental needs to the fullest extent. For example, you should be writing down any medications you are currently taking and noting any health conditions you believe we should know about. If you’d like to save time ahead of your initial appointment, please print and fill them out ahead at home and bring them to the office.
