Your First Visit
When you first arrive, you’ll be greeted by our friendly front desk team member. They will then hand you new patient forms to fill out. In a few minutes, one of our team members will call you in so they can take you to one of our operatories. You’ll soon meet Dr. Aptaker who will go over your new patient forms, answer questions you might have and perform a brief exam. You’ll also get to schedule your next visit before you leave.
New Patient Forms
Your new patient form gives our dental office a chance to get to know your needs better before the dentist sees you. Make sure to make note of any medications you are taking as well as any health conditions we should be aware of moving forward. You’ll also have the option to print and fill them out prior to your visit if you’d like to save time during the check-in process.

We accept cash, personal checks and credit cards (Visa, Mastercard, American Express, and Discover). Financing may also be available through CareCredit.
We also accept select insurance plans and we are pleased to assist you with processing your insurance claims. Even if we do not accept your insurance plan, we will work with you to maximize your benefits. Please bring the necessary information to your first visit. Insurance policies may not cover all of the treatment you chose to receive, and we ask that you pay your portion of the bill at the time of treatment. If you have questions, do not hesitate to ask.
